Tax exemption certificateIf applicable
A state-issued document showing that qualifying purchases for your organization are exempt from sales tax. You only need this if your business claims tax-exempt status.
EIN (Federal Tax ID), or SSNFor sole proprietors, if requested
An EIN is the nine-digit Federal Tax ID the IRS assigns to a business. If you’re a sole proprietor without an EIN, you may be asked for your Social Security number instead. Have your main contact’s name, email, and phone number ready too.
Billing and shipping addresses
Your billing address is the address connected to your business or payment records. Your shipping address is where you want orders and complimentary samples delivered; they can be the same address.
